SHirley's Story

Shirley Cotrell was in and out of the hospital for years with congestive heart failure. Her condition required ongoing monitoring, but she didn’t want to move to a long-term care facility. Shirley’s Care Coordinator at SummaCare arranged for home telemonitoring, allowing Shirley to monitor and report her weight, heart rate, blood pressure and more. Today, Shirley lives at home thanks to coaching and follow-up calls from her SummaCare Care Coordinator.
