Shirley's Story Shirley Cotrell was in and out of the hospital for years with congestive heart failure. Her condition required ongoing monitoring, but she didn’t want to move to a long-term care facility. Shirley’s Care Coordinator at SummaCare arranged for home telemonitoring, allowing Shirley to monitor and report her weight, heart rate, blood pressure and more. Today, Shirley lives at home thanks to coaching and follow-up calls from her SummaCare Care Coordinator. | |

The Pfeiffers' Story Because we make house calls, Jake and Pat Pfeiffer shared their 60th anniveary at home. Between the two of them, the Pfeiffers have endured a heart attack, a stroke, COPD and surgery. Pat was constantly in and out of the hospital, and Jake was too weak to take her to the doctor. Thanks to SummaCare's House Calls program, Pat and Jake now get regular home visits from a physician who monitors their conditions and medications. Pat hasn't been back to the hospital, and with greatly improved quality of life, they're able to live happily together in their own home. | |
